Kidney plays an integral role in the human body by maintaining overall fluid balance, filtering medications and toxic substances and regulating and filtering minerals from blood. When one suffers with renal disease then all of these normal functionalities get hampered and the need to undergo kidney transplant surgery arises.

Other than renal disease there are also other types of kidney diseases that can lead to dysfunctional consequences and failure in normal kidney functionality –

  • Chronically high blood pressure
  • Deep-seated problem in the anatomy of urinary tract
  • Glomerulonephritis
  • Diabetes mellitus and
  • Polycystic mellitus
kidney Transplantation Source

Kidneys that are selected during transplantation comes from two main sources-

Living Donor – When taking a kidney from a living donor, family members or close blood relatives of the patient are considered first. The donor must have a good health and should be well informed about the transplantation process; also the consent of the donor is needed.

Deceased Donor – The Uniform Anatomical Gift Act gives freedom to everyone for consenting to organ donation once they are deceased. Once the person suffers from brain death the kidneys are removed and stored until transplanted into a selected recipient.

Some of the common symptoms of kidney disease are-

  • General fatigue and shortness of breath.
  • Retention of fluid causing swelling (edema).
  • Foamy urine that is dark brown, orange, red or tea-colored.
  • Back pain.
  • Increased frequency of urination.
  • Metallic taste.
  • Restless legs and severe leg cramps.
  • Constant breathing problem due to the accumulation of urea in blood.
  • Loss of appetite.
  • Tingling sensations and numbness in the toes or fingertips.
  • Problems in concentrating

Steps before the transplant are -

  • You will get proper explanation to the entire procedure and you can ask your queries.
  • You will be required to sign consent before surgery.
  • You will be kept on dialysis before surgery.
  • Before surgery starts you will be asked to fast for 8 hours, generally starting after midnight.
  • You might get a sedative before the procedure starts to make you feel relaxed.
  • Further your medical team might prepare for some other specific preparations based on your medical condition.
Evaluation Process For Kidney Transplant

Mental Health Evaluation – This evaluation is conducted on both the recipient as well as the donor. This test analyses the extent of some of the major Social and psychological issues such as stress, support by the family and financial issues. These type of issues can hamper the final outcome of the transplant.

Blood Tests- Blood tests are mandatory in this situation and are done to find a perfect donor that is a good match for the recipient.

Blood Group Testing - It is very important test to detect patient’s blood type. The four blood types are-A, B, AB and O and everybody have got one of these four blood types. In the case of kidney transplantation the recipient and the donor should either have the same blood type or a compatible one.

The blood type that very easily matches with all other blood types is the AB blood type as the AB blood type people are universal acceptors. The hardest blood type to match is the O blood type as these people are universal donors and can donate the kidney to anybody but are not universal acceptors.

Diagnostic Tests- These tests are conducted to check the overall health of the donor as well as of the recipient. The tests include ultrasound, X-rays and kidney biopsy. For women patients some more tests are included such as gynecology evaluation, Pap test and a mammogram.

Tissue Typing - Tissue typing test which is also a form of blood test but is done to detect the human leukocyte antigens (HLA). This test is mandatorily performed on both the recipient and the donor during the process of evaluation. The transplants that are done with a perfect match have got higher success rate and better chances to last longer. The highest number of “perfect match” kidney transplant case has been noted to come from the siblings.

Crossmatch - In the crossmatch test the doctor mixes the recipients blood with that of the donor’s cells. If the result of the crossmatch test is positive then it means that the present antibodies are against the donor. In this case the recipient should not receive that particular kidney until and unless certain treatments are conducted to reduce the level of antibodies.

Serology - Some tests are even conducted to detect viruses such as hepatitis, HIV and CMV (cytomegalovirus) so that proper preventive medications can be supplied right after the transplant.
